Physostigmine is a cholinesterase inhibitor belonging to the carbamate class. It is also referred to as an indirectly acting cholinomimetic because, by inhibiting acetylcholinesterase, it potentiates responses to endogenous acetylcholine, which is released from cholinergic nerve endings in the central nervous system and periphery.

PHYSOSTIGMINE FOR PROMETHAZINE POISONING

Experience suggests that the central anticholinergic action of promethazine is a major element in the toxic effects following overdosage. Physostigmine seems to be a direct antidote at doses within the safe therapeutic range; side-effects, if they become a problem, can be treated with intravenous atropine.
