Unleashing rapamycin in fibrosis
Rapamycin (sirolimus, rapamine) is a macrocyclic antibiotic produced by the bacteria streptomyces hygroscopicus, with immunosuppressive and antiproliferative capabilities. Rapamycin binds with FK- binding protein 12 to inhibit the mechanistic (formerly mammalian) Target of Rapamycin (mTOR) protein [1].

This review delineates the hypothalamic circuits that control liver homeostasis via autonomic and neuroendocrine pathways. Dysregulation of this hypothalamus–liver axis drives disease progression across a spectrum including steatotic liver disease, liver inflammation and injury, fibrosis, cirrhosis, and hepatocellular carcinoma.

In diabetic kidney disease, elevated podocyte ANGPTL4 is linked to reduced TFEB nuclear localization and compromised lysosomal degradative function. These changes impair podocyte lipophagy and promote lipid‐droplet accumulation and podocyte injury, which may contribute to renal injury progression.

VEGFR‐2 signaling in OSCC activates Src–STAT6‐dependent CSF2 transcription, driving tumor‐derived GM‐CSF secretion. GM‐CSF programs neutrophils to express PD‐L1 through STAT5–mTOR/S6K signaling, suppressing cytotoxic CD8+ T cells. This pathway reveals a tumor–neutrophil immune checkpoint circuit that limits anti‐PD‐1 responsiveness in OSCC.
