病例资料患者,男,86岁,因"腰酸不适40余年,浮肿加重伴皮肤瘙痒脱屑2周",于2020年3月15日收入南京市中医院肾内科。患者40余年前无明显诱因下出现腰酸不适,于外院诊断为"慢性肾炎综合征",间断服用中药治疗症状稍改善,未系统监测肾功能。自诉近年来血肌酐偏高(具体不详),蛋白尿波动在1+~2+。4周前因高磷血症加用碳酸司维拉姆,每次口服800 mg、3次/d降磷,服药2周后出现皮肤脱屑,瘙痒加重等症状,在我院皮肤科经诊断考虑为过敏。
