创伤后骨关节炎（PTOA）是关节损伤后的常见远期结局之一，其发生发展与持续性炎症、细胞衰老及软骨基质代谢失衡等相关。影像学检查对早期PTOA改变的提示作用有限，滑液外泌体miRNA因稳定性较好、局部特异性较强而受到越来越多的关注。本文对滑液外泌体miRNA在PTOA中的异常表达及其与炎症调控、细胞衰老和软骨代谢相关的机制进展作一综述，以期为PTOA早期诊断、病程监测及临床转化提供参考。
