ABSTRACT Leptospirosis can masquerade as acute cholangitis with cholestatic jaundice, fever, and leukocytosis. In febrile patients with cholestatic liver profile without biliary obstruction, especially farmers with calf pain and conjunctival suffusion, consider leptospirosis early to initiate timely antibiotics and prevent poor outcomes.
