Comparison of Remimazolam and Propofol for Sedation in Endoscopic Retrograde Cholangiopancreatography: A Systematic Review and Meta‐Analysis With Trial Sequential Analysis

open access: yesDigestive Endoscopy, EarlyView.
This systematic review and meta‐analysis of 5 randomized controlled trials (n = 965) compares the safety profile of remimazolam versus propofol for sedation during ERCP. Remimazolam was associated with a significantly lower incidence of hypoxia, hypotension, and bradycardia.

Effectiveness and safety of abobotulinumtoxinA in pediatric lower limb spasticity: A phase IV, prospective, observational, multicenter study

open access: yesDevelopmental Medicine &Child Neurology, EarlyView.
Abstract Aim To assess the longitudinal attainment of patient‐centered, function‐related Goal Attainment Scaling Total (GAS T)‐score after repeated abobotulinumtoxinA (AboBoNT‐A) injections over a period of up to 30 months and up to 10 cycles. Method In this prospective observational study, the investigators' clinical practices recruited patients aged ...

Can the large language model ChatGPT‐4omni predict outcomes in adult patients with status epilepticus?

open access: yesEpilepsia, Volume 66, Issue 3, Page 674-685, March 2025.
Abstract Objective Large language models (LLMs) have recently gained attention for clinical decision‐making and diagnosis. This study evaluates the performance of the recently updated LLM Chat Generative Pre‐Trained Transformer‐4omni (ChatGPT‐4o) in predicting clinical outcomes in patients with status epilepticus and compares its prognostic performance

Community‐onset pediatric status epilepticus: Barriers to care and outcomes in a real‐world setting

open access: yesEpilepsia, Volume 66, Issue 3, Page 725-738, March 2025.
Abstract Objective Status epilepticus (SE) is a neurological emergency in childhood, often leading to neuronal damage and long‐term outcomes. The study aims to identify barriers in the pre‐hospital and in‐hospital management of community‐onset pediatric SE and to evaluate the effectiveness of pediatric scores on outcomes prediction.

Synaptic inhibitory dynamics drive benzodiazepine response in pediatric status epilepticus

open access: yesEpilepsia, EarlyView.
Abstract Objective Pediatric status epilepticus (SE) is a medical emergency associated with significant morbidity. Benzodiazepines (BZPs) are the current first‐line treatment, but do not work in more than one third of children presenting with SE. Animal studies have shown that SE can cause changes in synaptic inhibition signaling that can ultimately ...

Changes in cortical delta power during chronic invasive epilepsy monitoring

open access: yesEpilepsia, EarlyView.
Abstract Objective Cortical delta band (1–4 Hz) activity is considered a biomarker for states of altered consciousness, with increased delta power observed during anesthesia, sleep, coma, and delirium. The current study sought to characterize delta power following electrode implantation with respect to patient demographics and clinical characteristics ...

The human photosensitive epilepsy model for clinical proof‐of‐principle trials of novel antiseizure medications: 2. Analysis of drug trials and predictive value of the model

open access: yesEpilepsia, EarlyView.
Abstract Clinical development of novel antiseizure medications (ASMs) would benefit from an early proof of principle (POP) model. The photosensitivity model, which uses the photoparoxysmal electroencephalographic response (PPR) as a surrogate of seizures, is currently the only human model that allows POP trials of investigational compounds after a ...

Treatment of Benzodiazepine Dependence

New England Journal of Medicine, 2017
Long-term use of benzodiazepines can lead to dependence. Symptoms of withdrawal include anxiety, irritability, confusion, seizures, and sleep disorders. Withdrawal management relies on the use of a single agent (diazepam) and gradual dose reduction.

Is Benzodiazepine Use Associated With the Risk of Dementia and Cognitive Impairment–Not Dementia in Older Persons? The Canadian Study of Health and Aging

The Annals of Pharmacotherapy, 2020
Background: The use of benzodiazepines in relation to cognitive decline remains an area of controversy in aging populations. Objective: This study aims to evaluate the risk of cognitive impairment–not dementia (CIND), Alzheimer disease (AD), and all ...
