Mechanism for development of systemic endotoxemia in patients with cirrhosis and portal endotoxemia
肝硬変患者の末梢エンドトキシン血症出現の機序を知る為に,肝硬変20例,非硬変7例に経皮経肝カテーテル法を用い門脈系(門脈本幹,上腸間膜静脈,脾静脈)と末梢の血液を同時に採血しエンドトキシンを測定した.門脈系血でのエンドトキシン検出率は,肝硬変,非硬変例共に上腸間膜静脈血で高く,エンドトキシン濃度は,肝硬変例の方が非硬変例に比して高い傾向にあった.門脈系血中エンドトキシン陽性の肝硬変例のうち末梢での血中エンドトキシン陽性群は,陰性群に比してICG 15分停滞率の高値,肝内短絡路の増加がみられた.即ち肝硬変患者の末梢エンドトキシン血症の出現は,有効肝血流量の減少に起因する肝の細網内皮系機能低下により門脈血液中のエンドトキシンの除去ができないためと考えられた.
