Idiopathic focal segmental glomerulosclerosis (FSGS) is a primary glomerular disease that essentially represents a form of chronic, progressive renal fibrosis for which there is no discernible cause. Often presenting with or eventually manifesting the nephrotic syndrome, this disease is increasing in incidence in both children and adults.

• We studied 100 renal biopsy specimens from adults with the primary nephrotic syndrome in an inner city hospital serving mostly black patients and found that 47 had focal segmental glomerulosclerosis. Most of the men presented in the third decade of life, a peak distribution not seen in women.

Focal and Segmental Glomerulosclerosis

Focal and segmental glomerulosclerosis (FSGS) is a common cause of Nephrotic Syndrome in adults. Its most typical course runs from NS to progressive loss of renal function and may recur in the graft after renal transplantation. Current evidence favors prolonged corticosteroid therapy (6 months or longer) to induce remission of proteinuria.
