耐糖能障害―肥満・メタボリックシンドローム・糖尿病―は,認知障害や認知症のリスクファクターである.医師を対象とした認知症診療に関するアンケート調査では,生活習慣病を有する高齢者では,認知症が見逃されやすいことが示された.認知症の早期発見には,医師のみならずケアスタッフの教育や認知症を積極的に疑うことが重要と考えられる.高齢者糖尿病の脳を守るために,代謝性脳症予防の視点からは,随時血糖を270 mg/dl 以下に管理することは合理的な目標値と考えられる.アルツハイマー型認知症の動物実験モデルでも,神経細胞障害を抑制するために,高血糖・低血糖,急峻な血糖変動を避けることが重要であった.今後,アミロイド神経病変の抑制,ADLの維持という視点から,更なる具体的な血糖管理目標値が提唱されるべきである.
