A CASE OF SPONTANEOUS COMPLETE NECROSIS OF HEPATOCELLULAR CARCINOMA
自然壊死をきたした稀な肝細胞癌症例を経験したので報告する.症例は71歳の男性. C型慢性肝炎の診断で近医通院中であった.肝S5領域に腫瘤性病変を認め,肝細胞癌の診断で当科へ紹介入院の後,肝S5の部分切除術を施行した.肉眼的に病変は線維組織で取り囲まれた黄色の結節であり,病理学的所見では内部の組織は完全に壊死に陥っていた.術前の画像所見と腫瘍マーカーの推移に加え,標本の鍍銀染色に細網線維の残存が観察されたことや,抗ヘパトサイト抗体の陽性像などより,この壊死組織は自然壊死をきたした肝細胞癌であると考えられた.その機序として,血管造影に伴う内膜損傷による肝への血流の低下,免疫学的反応の関与が考えられた.文献上,同様な症例は6例であった.
