Imprint cytology of testicular carcinoid.

背景:まれな精巣カルチノイドの1例を経験したのでその捺印細胞所見を中心に報告する.症例:症例は64歳, 男性.肉眼的血尿を主訴とし当院受診となった.左精巣に3cm大の腫瘤を触知, 画像検査にて精巣腫瘍を疑い左高位除睾術を施行した. 捺印細胞像としては, 腫瘍細胞は孤立散在性あるいは疎な結合を示す平面的な集団として認められた.核は円形でクロマチンは細穎粒状で均等に分布したものが多く, 裸核の細胞や, 細穎粒状の豊富な細胞質を持つ細胞が多数認められた. こうした所見よりセミノーマあるいはカルチノイドが疑われたが確定には至らなかった. その後, 組織学的な検索によりカルチノイドと診断された.結語:まれな精巣カルチノイドを経験した.

目的 : 下垂体腫瘍に対し捺印細胞診を行い, 腺腫細胞の特徴を明らかにし, 術中迅速診断に有用であるか検討を行った.方法 : 下垂体腺腫摘出 94 例から術中迅速組織診検体を用いて捺印標本を作製し, その組織像と比較検討した.成績 : 下垂体腺腫 86 例 (91.5%) で下垂体腺腫の確認が可能であった. また, 組織亜型分類では, fibrous body や分泌顆粒などの特徴的な細胞内構造物に加え, 細胞形状, 細胞結合性, 核の位置などの細胞所見を総合的に捉えることによって, 3 群 (GH-PRL-TSH 群, ACTH 群, FSH-LH/ナルセル群) に分類可能であった.結論 : 下垂体腺腫の捺印細胞診は, 腺腫細胞の特徴を十分に捉えることが可能であり ...
