Consensus Recommendations for Clinical Pharmacist Integration into the Acute Stroke Care Team: Endorsed by the American College of Clinical Pharmacy, Neurocritical Care Society, Society for Academic Emergency Medicine, and Society of Critical Care Medicine, and affirmed by the American Academy of Neurology

open access: yesJACCP: JOURNAL OF THE AMERICAN COLLEGE OF CLINICAL PHARMACY, Volume 9, Issue 7, July 2026.
Graphical abstract of consensus recommendations for clinical pharmacist integration into the acute stroke care team. ABSTRACT Clinical pharmacists are increasingly recognized as vital contributors to acute stroke care, yet their role remains inconsistently defined and underutilized across health care systems.

Ischemia–Reperfusion Injury: Molecular Mechanisms and Therapeutic Interventions

open access: yesMedComm, Volume 7, Issue 7, July 2026.
Multiorgan ischemia–reperfusion injury begins with ischemia‐induced ATP depletion and ionic imbalance, followed by reperfusion‐triggered mitochondrial ROS/RNS bursts, regulated cell death, and DAMP release. Sterile inflammation converges on endothelial–immune–coagulation crosstalk, where NETs drive immunothrombosis, no‐reflow, and remote organ injury ...

Influence of Decision‐Makers' Perceived Risk on the Propensity for Intravenous Thrombolysis in Acute Ischemic Stroke: A Cross‐Sectional Study

open access: yesNursing Open, Volume 13, Issue 7, July 2026.
ABSTRACT Aims This study investigates the degree of the perceived risk of intravenous thrombolysis in acute cerebral infarction among surrogate decision‐makers and the factors influencing their propensity to make decisions about thrombolysis. Design An investigation was conducted on a cross‐sectional basis.

Direct Thrombectomy vs. Combined Treatment With Intravenous Thrombolysis in the Extended Time Window: A Target Trial Emulation

open access: yesEuropean Journal of Neurology, Volume 33, Issue 7, July 2026.
This target‐trial emulation compared direct MT (n = 212) vs. IVT + MT (n = 135) in anterior‐circulation LVO patients treated 4.5–24 h or at wake‐up, eligible for IVT with perfusion mismatch. Using IPW with a non‐inferiority margin RD ≥ −1.3% for 90‐day mRS 0–2, direct MT was not non‐inferior to IVT + MT (adjRD −2.9%, 95% CI −6.64 to 0.84).

Neuroimaging biomarkers in migraine with aura mimicking stroke: A qualitative and quantitative analysis of perfusion CT alterations

open access: yesHeadache: The Journal of Head and Face Pain, Volume 66, Issue 7, Page 1607-1616, July/August 2026.
Plain Language Summary Migraine with aura can mimic a stroke, making diagnosis difficult and sometimes leading to unnecessary treatment in the acute setting. We found that some patients with migraine with aura show characteristic changes on brain perfusion CT scans, particularly in men and those with speech symptoms, which can be better detected using ...

Blood Pressure Variability in Acute Ischemic Stroke: Current Evidence, Measurement Challenges, and Future Directions

open access: yesThe Journal of Clinical Hypertension, Volume 28, Issue 7, July 2026.
ABSTRACT Blood pressure variability (BPV) has emerged as a prognostic hemodynamic marker in acute ischemic stroke (AIS), with observational studies consistently associating early systolic fluctuations with infarct progression, neurological deterioration, hemorrhagic transformation, and worse functional outcomes.

Prediction of incident heart failure in type 2 diabetes mellitus: Rationale and design of the DM‐HEART study

open access: yesJournal of Diabetes Investigation, Volume 17, Issue 7, Page 1154-1162, July 2026.
ABSTRACT Preclinical heart failure (HF) (stages A and B) is common but easily overlooked in asymptomatic patients with type 2 diabetes (T2D). There is a paucity of evidence regarding how coexisting risk factors contribute to the development of HF in asymptomatic Asian patients with T2D, and there is a lack of guidance on the early identification of ...

Intravenous thrombolysis for acute stroke.

open access: yesCleveland Clinic Journal of Medicine, 2004
Intravenous t-PA is effective if given to appropriate patients within 3 hours of stroke onset, and its effectiveness increases even within the first 3 hours when given as soon as possible. t-PA is reasonably safe if used in a carefully defined manner that ensures close attention to blood pressure, careful patient monitoring, no use of heparin and ...

Branch retinal artery occlusion due to patent foramen ovale in a teenager

open access: yesExperimental Physiology, Volume 111, Issue 7, Page 3173-3178, 1 July 2026.
Abstract This report demonstrates the diagnostic evaluation and clinical management of a 16‐year‐old girl who developed branch retinal artery occlusion secondary to patent foramen ovale. The goal is to increase awareness of this rare presentation in adolescents among clinicians, with a summary of the important clinical features, multimodal imaging ...

Clinical and epidemiological overview of hyperacute ischemic stroke patients treated with intravenous thrombolysis

open access: yesThe Egyptian Journal of Internal Medicine
Background Hyperacute ischemic stroke is a type of ischemic stroke that occurs rapidly and suddenly due to blockage of blood flow to the brain, with onset within 0–24 h.
