Abstract Background Aging is accompanied by a chronic low‐grade inflammatory process, known as inflammaging, as well as immunosenescence, an age‐related decline and dysregulation of immune function, and cellular senescence, a process in which cells enter a state of irreversible growth arrest while actively releasing pro‐inflammatory factors.

The Role of EVs in the Aging Hematopoietic System
Extracellular vesicles from aged bone marrow carry pro‐inflammatory cargo that drives blood stem cell decline, while youthful vesicles restore or maintain stem cell function. Circulating vesicles therefore represent both a readout of hematopoietic aging and a potential therapeutic route.

Using 38 blood SASP proteins, a guided autoencoder‐transformer model generates a composite SASP Score that captures systemic cellular senescence burden. The score predicts mortality and age‐related diseases, outperforms individual proteins, and supports cross‐platform biomarker translation.
