Perilymph fistula occurs when endolymph and perilymph mix, or when perilymph leaks into the middle ear space. Sensorineural hearing loss and/or vertigo may result. This paper reviews the pertinent anatomy and physiology of the inner ear, clinical presentations, diagnosis, treatment, and prognosis as a guide for the clinician.

Occupational perilymph fistula
American Journal of Industrial Medicine, 1994AbstractA case of occupational perilymph fistula in an ironworker is described. The patient was lifting and lowering 200 pounds of steel when he developed sudden onset of vertigo. Surgical exploration of the middle ear demonstrated leakage of perilymph fluid from the round window.

The Laryngoscope, 1980
AbstractBetween July 1976 and August 1979, 14 patients underwent surgical repair of a traumatic tympanic membrane perforation. Of these 14 patients, 9 were found to have a profuse perilymph fistula of the round and/or oval window. The perilymph fistula involved the oval window in 2 patients, the round window in 2 patients, and both the oval and round ...

Perilymph fistula pathophysiology
Otolaryngology–Head and Neck Surgery, 1995A great deal of the controversy associated with perilymph fistula is due to the lack of a sensitive, specific way of detecting and thus diagnosing one. The existing subjective, clinical observations need to be replaced with an objective, scientific methodology having higher specificity and sensitivity for the detection of perilymph fistula.

Perilymph Fistula: The Stanford Experience
Annals of Otology, Rhinology and Laryngology, 1988Seventy-eight tympanotomies were performed to determine the presence of perilymph fistulas (PLFs); of these, 51 % were found. The oval and round windows of all patients were grafted, whether PLFs were present or not. Of those patients with PLFs, 64 % had resolution of their major symptom; when no PLF was found, 44% had a similar outcome.

Fifty-one patients suspected of having a perilymph fistula were evaluated. We postulated that many patients with predominantly vestibular complaints had unrecognized perilymph fistulas. An analysis was made of symptoms, physical findings, vestibular and audiometric test results in order to determine appropriate diagnostic criteria for the presence of ...

The Laryngoscope, 1981
AbstractA perilymph fistula is an abnormal communication between the fluids surrounding the membranous labyrinth and the middle ear space. Because of the potential hazards of meningitis, permanent hearing loss, and occasionally incapacitating vestibular symptoms, early recognition and prompt repair of the perilymph leak is important. One hundred thirty ...

Perilymph fistula—a diagnostic dilemma
The Journal of Laryngology & Otology, 1991AbstractA retrospective series is presented of 51 cases operated on for suspected perilymph fistula. In 26 ears a fistula was identified at surgery. A positive fistula test was found to strongly indicate a perilymph fistula but was more often negative than positive in surgically demonstrated fistula ears.

The Tullio Phenomenon and Perilymph Fistula
Otolaryngology - Head and Neck Surgery, 1988Establishing the diagnosis of perilymph fistula remains a dilemma. At this time, identification of an active perilymph fistula can be confirmed only by surgery. On the basis of clinical history and audiovestibular testing, 54 patients underwent middle ear exploration for possible perilymph fistula at the Colorado Ear Clinic between July 1980 and June ...
