Pharmacokinetics of Phenformin in Man

Phenformin was assayed in urine, plasma, and sputum specimens, obtained from two healthy volunteers during the four-day period following oral administration of a single therapeutic dose. Approximately one third of the drug was excreted unchanged in the urine.

PHENFORMIN AND METABOLIC ACIDOSIS

Abstract A patient with oliguric renal failure complicating accelerated hypertension was given phenformin and ethylœstrenol as fibrinolytic therapy. On the fifth day of this treatment a profound and fatal metabolic acidosis developed despite good control of uraemia by peritoneal dialysis. The acidosis was resistant to 2500 meq.

Phenformin and lactic acidosis

There is considerable evidence, both from animal studies and clinical reports, to suggest that the hypoglycemic agent phenformin interferes with lactate metabolism. Experimental data are reported which are consistent with a defiCiency in the ability of phenformin‐treated rats to handle endogenous and exogenous lactate.

Abstract:The metabolic effects of phenformin and phenformin‐ethanol were studied in isolated perfused liver preparations from fed rats. Whether phenformin was given alone or together with ethanol, the hepatic oxygen consumption decreased and the lactate production increased. Phenformin caused a decrease in ethanol elimination.
