This study developed and internally validated a nomogram incorporating baseline NIHSS score, monocytes, platelets, post‐thrombectomy fasting blood glucose, and number of passes to predict parenchymal hematoma after EVT in large core infarctions. A pass‐by‐IVT interaction suggests minimizing unnecessary passes during direct thrombectomy.
