Dual‐Targeted M1 Macrophage Extracellular Vesicles Reprogram the Tumor Microenvironment and Enhance Natural Killer Cell Immunotherapy in Breast Cancer

Dual‐targeted extracellular vesicles (EV) engineered from M1 macrophages (M1EV_HA/cRGD) bridge NK cells to breast cancer cells while activating pro‐inflammatory programs within the tumor microenvironment. Hyaluronic acid (HA) enables CD44‐mediated NK targeting, and cRGD binds tumor integrins for targeted dual engagement.

This prospective single‐arm study evaluated neoadjuvant disitamab vedotin plus penpulimab in 20 patients with Stage II–III HER2‐low breast cancer. The regimen showed preliminary activity (pCR 20.0% intention‐to‐treat, 25.0% evaluable; ORR 56.3%) with manageable toxicity.
