Every HLA antigen, as defined by the WHO-HLA Nomenclature, is unique. The major function of these molecules is to present antigen-peptides to the T-cell receptor, thereby contributing to the immunological defence mechanism. This function is regulated for each MHC antigen by its unique structure, with the peptide-binding pockets of the three-dimensional

Immunogenetics, HLA-B27 and spondyloarthropathies
Current Opinion in Rheumatology, 1999HLA-B27 is the strongest HLA molecule associated with a disease. However, the reason only a small fraction of HLA-B27 positive individuals develop spondyloarthropathies is still unknown. Recent advances in genetics support the fact that additional genetic factors influence the disease and that the environmental factors may be ubiquitous.

The various subtypes of HLA-B27 are described together with their worldwide distribution. None is uniquely related to AS but the evidence favours a direct role for B27 in the disease pathogenesis.

Neutrophil Function and HLA-B27
Rheumatology, 1983The pathogenesis of HLA-B27 linked diseases, including reactive enteroarthritides, is not known. Differences in immune response, cross-reaction between microbial antigens and host cell structures, attachment of microbial components on the host cell surfaces with subsequent autoimmune reaction, and inflammatory responsiveness of the host may be involved.

HLA-B27--associated acute anterior uveitis, a relatively common form of uveal inflammation, has a sufficiently characteristic presentation that the diagnosis is often suggested by clinical assessment, although testing for HLA-B27 status provides strong support for the diagnosis. This condition usually responds readily to topical corticosteroid therapy,

Non-HLA-B27 Genetic Factors in HLA-B27 Associated Diseases
Clinical Rheumatology, 1996Family and twin studies showed that besides HLA-B27 other genetic factors are of significance for the pathogenesis of ankylosing spondylitis, reactive arthritis or acute anterior uveitis. There are up to now no strong indications that any subtype of B27 is especially associated with these diseases.

Outcomes of HLA-B27-Positive and HLA-B27-Negative Acute Anterior Uveitis
American Journal of Ophthalmology, 1995Outcomes of HLA-B27-positive and HLA-B27-negative acute anterior uveitis were assessed after a mean follow-up of nine years. Rheumatologic complications, in particular the presence and course of ankylosing spondylitis, were examined during the same period.A hospital-based prospective study of 119 patients with HLA-B27-positive and 35 patients with HLA ...

Genetical Factors—other than HLA-B27—in “HLA-B27-associated diseases”
Scandinavian Journal of Rheumatology, 1990All studies among acute anterior uveitis (AAU) and ankylosing spondylitis (AS) agree on the importance of and high association with HLA-B27. However, the majority (greater than 95%) of the HLA-B27+ population will never develop such disease. There are two--generally accepted--important factors increasing the relative risk for developing AAU or AS ...

HLA-B27-Associated Cardiac Disease
Annals of Internal Medicine, 1997To review the available information about cardiac disease in relation to the immunogenetic marker HLA-B27 and the inflammatory disorders associated with it (seronegative spondyloarthropathies).English-language clinical studies, case reports, and reviews published in 1995 or earlier were identified from a search of the MEDLINE database.
