Persistent atrial fibrillation, frequent premature ventricular complexes, and limited tolerance of guideline‐directed medical therapy contributed to progressive ventricular dysfunction, with LVEF declining from approximately 50% to 20%–25%. AV nodal ablation with CRT‐D implantation provided durable ventricular rate control and biventricular pacing.

Although Pulsed field ablation (PFA) has demonstrated efficacy for atrial fibrillation, its safety in patients with leadless pacemaker remains uncertain.
