Meldonium functions as an inhibitor of γ-butyrobetaine hydroxylase, the enzyme responsible for catalyzing the biosynthesis of L-carnitine. By inhibiting mitochondrial β-oxidation of fatty acids under hypoxic conditions, meldonium exerts cytoprotective effects on cardiac, hepatic, and pulmonary tissues.
