Methadone and edema

We describe a patient on methadone maintenance who developed distal leg edema after years of treatment. The edema resolved when methadone was discontinued and recurred when it was restarted. This patient is compared to three others reported in the literature who also developed fluid retention shortly after being placed on methadone.

Detection of Methadone, LAAM, and Their Metabolites by Methadone Immunoassays [PDF]

l-Alpha-acetylmethadol (LAAM) was recently approved as a substitute for methadone. LAAM, methadone, and their common metabolite, methadol, are extensively N-demethylated. The structural similarities of LAAM and its metabolites to methadone suggest that they may cross-react in methadone immunoassays.
