Total Phenytoin concentration is not well correlated with active free drug in critically-ill head trauma patients

open access: yesJournal of Research in Pharmacy Practice, 2013
Objective: Phenytoin is an antiepileptic drug used widely for prophylaxis and treatment of seizure after neurotrauma. Phenytoin has a complex pharmacokinetics and monitoring of its serum concentrations is recommended during treatment.

The Cerebellum and SIDS: Disordered Breathing in a Mouse Model of Developmental Cerebellar Purkinje Cell Loss during Recovery from Hypercarbia. [PDF]

open access: yes, 2016
The cerebellum assists coordination of somatomotor, respiratory, and autonomic actions. Purkinje cell alterations or loss appear in sudden infant death and sudden death in epilepsy victims, possibly contributing to the fatal event. We evaluated breathing

The sodium channel-blocking antiepileptic drug phenytoin inhibits breast tumour growth and metastasis

open access: yesMolecular Cancer, 2015
BackgroundVoltage-gated Na+ channels (VGSCs) are heteromeric protein complexes containing pore-forming α subunits and smaller, non-pore-forming β subunits. VGSCs are classically expressed in electrically excitable cells, e.g. neurons.

Targeting ROCK2 to Restore Epileptic Synaptic Networks via Mitophagy Activation: Insights from Translational Imaging of SV2A In Vivo

open access: yesAdvanced Science, EarlyView.
By integrating synaptic vesicle glycoprotein 2A PET imaging and transcriptomics, this study identifies Rho‐associated protein kinase 2 (ROCK2) as a critical regulator of synaptic network dysfunction in temporal lobe epilepsy. Pharmacological ROCK2 inhibition restores synaptic density and enhances mitophagy, thereby reducing seizures and neuronal damage.

Refractory Status Epilepticus and Therapeutic Uncertainties: a Comprehensive Review on Targeting Neuroinflammation and Rationale for Developing a Platform Trial

open access: yesAdvanced Science, EarlyView.
Refractory status epilepticus (RSE) is a life‐threatening neurological emergency with high rates of death and long‐term disability, but treatments for it are often based on limited evidence. RSE is now understood to involve neuroinflammation, which has led to the exploration of anti‐inflammatory and immunomodulatory agents like Tocilizumab. Traditional

Fetal hypoxia and hyperglycemia in the formation of phenytoin‐induced cleft lip and maxillary hypoplasia

open access: yesEpilepsia Open, 2019
Objective Phenytoin exposure during the first trimester of pregnancy increases the risk of maxillary hypoplasia and cleft lip. The etiology of phenytoin embryopathy is unknown.

Management of epilepsy [PDF]

open access: yes, 1989
Figures for the incidence of epilepsy in Malta are not available. The overall figure for epilepsy given by the Royal College of General Practitioners (Reid 1960) is 4.82 per 1,000 population.

Mutations in the Key Autophagy Tethering Factor EPG5 Link Neurodevelopmental and Neurodegenerative Disorders Including Early‐Onset Parkinsonism

open access: yesAnnals of Neurology, EarlyView.
Objective Autophagy is a fundamental biological pathway with vital roles in intracellular homeostasis. During autophagy, defective cargoes including mitochondria are targeted to lysosomes for clearance and recycling. Recessive truncating variants in the autophagy gene EPG5 have been associated with Vici syndrome, a severe early‐onset neurodevelopmental
