Summary Timolol maleate is a new, potent ocular antihypertensive that has an effect additive with other antiglaucoma medications. It does not cause miosis and is relatively free of ocular complications, although a few have been documented. Timolol does exacerbate asthma and should not be used in patients with a history of this disease.

A 56-year-old woman with an open angle glaucoma was treated with timolol maleate topically for 6 months. The patient developed peculiar nail pigmentations of the fingers and toes which could not be related to vasospastic disorders. After withdrawal of the timolol therapy normal-colored nails grew.

Corneal Anesthesia after Timolol Maleate Therapy

Symptomatic superficial punctate keratitis associated with complete corneal anesthesia occurred in three patients after they had been taking timolol maleate for glaucoma. After discontinuation of the timolol maleate and substitution of conventional antiglaucomatous collyria, the keratitis gradually cleared with restoration of normal corneal sensitivity.
