患者は55才の男性でショック状態で入院となった.入院時の体温は41°Cで脈拍は触知せず,眼瞼結膜の充血著明で躯幹の紅皮症,上腹部圧痛および筋痛がみられた.検査所見は急激な重症感染症,腎不全,筋障害,肝障害を示唆した.敗血症性ショックを疑い血液培養,尿培養,便培養,髄液培養,咽頭培養を頻回に行なったが全て陰性であった.抗生物質,コルチコステロイド,ドーパミン,ドブタミン,ノルアドレナリン等の薬物投与により,患者は回復し約1カ月の経過で後遺症を残さず退院した.症状および検査所見から本例はtoxic shock syndrome (TSS)の診断基準を十分に満足した.また血中よりTSST-1をRPLA法で証明した.診断基準を満足し,ブドウ球菌は培養にて陰性で,かつ血中より直接TSST ...
