Summary We describe a pregnant patient who presented in labour with diffuse ST‐segment elevation on electrocardiography and biochemical evidence of myocardial injury. Transthoracic echocardiography showed severe biventricular dysfunction. An urgent caesarean birth was indicated due to breech presentation and severe cardiac dysfunction.
