CST subfiber NDI values were reduced in prediagnostic ALS patients before clinically evident typical upper motor neuron signs emerged. Combining serum NfL with CST subfiber NODDI metrics showed higher diagnostic performance than single markers for differentiating prediagnostic ALS from healthy and ALS‐mimic controls.
