Undetectable CSF hypocretin-1 in “Hashimoto’s encephalopathy” associated with coma

Hypersomnolence has been reported in 40% of patients with steroid-responsive encephalopathy associated with autoimmune thyroiditis (SREAT),1,2⇓ sometimes termed “Hashimoto’s encephalopathy.”3,4⇓ We present a patient with SREAT and hypersomnolence and then coma who while symptomatic had undetectable levels of hypocretin-1 in the CSF.

Normalization of hypocretin-1 in narcolepsy after intravenous immunoglobulin treatment

In May 2006, a 28-year-old woman abruptly experienced excessive daytime sleepiness (EDS) and 2 to 3 short, refreshing but irresistible naps per day. She also reported increased nighttime sleep duration of more than 3 hours. Her Epworth Sleepiness Scale score was 22, indicating severe sleepiness.

Hypocretins/orexins are newly identified peptides of hypothalamic origin. Hypocretin deficiency is involved in the sleep disorder narcolepsy, suggesting the importance of hypocretin neurotransmission for the regulation of sleep.
