Clinical Case Reports, Volume 14, Issue 2, February 2026.ABSTRACT Acute acalculous cholecystitis can rarely be the initial manifestation of systemic lupus erythematosus. Awareness of this atypical presentation is essential to avoid unnecessary surgical intervention. Prompt recognition and initiation of immunosuppressive therapy may resolve symptoms and help establish the correct underlying diagnosis.
