大腸癌肝転移症例において肝転移巣に対し種々の治療を試み,その効果について検討した.1980年～1991年の12年間に当教室で経験した大腸癌肝転移症例は91例(同時性78例,異時性13例)で,これらに対する治療のうちわけは,リザーバーの留置による動注化学療法17例,動脈塞栓術(TAE)3例,肝切除術14例,大腸原発巣のみ切除し全身化学療法を併用したもの52例,試験開腹術5例であった.H1症例に対しては肝切除術はきわめて有効な手段であると考えられた.また肝切除術の対象とならないH3症例に対しては,リザーバーを設置しての間歇的反復動注化学療法(Epi-ADR 30mg,MMC 4mgを交互に1回/週)は全身化学療法より肝転移巣に対しての直接効果が高く有効な治療法と考えられ,また術後のlife ...
